Vulnerable and At-Risk Populations
There are separate criteria for your initial DQ and peer responses so please see below for directions. The peer posts are not what you are used to so please make sure you read fully before answering.
Chapters 14-18 in Harkness and DeMarco (2016) are great resources.
First, choose a Vulnerable or At-Risk population from the choice poll link and answer the following questions in this week’s initial DQ keeping that chosen population in mind.
For your INITIAL DQ-Place yourself in the role of the PCHN and discuss the following 4 criteria
1.Discuss what makes this particular population vulnerable? Remember, this is the population from the poll on the course home page. 1 paragraph (150 words)
2.Cite pertinent (local, state, and national) statistical information on your chosen population. This is not about the demographics in general; it is about your population that you chose in the poll and needs data from all 3 sources. Local is defined as the zip code or school district where your practicum facility is located. Make sure you say specifically where your facility is located as your peers will need it for their responses.1 paragraph (200 words)
3.What health issues are your chosen population at higher risk for? Explain why and how this impacts them. For example, the LGBT population is at higher risk for cardiovascular disease.1 paragraph (200 words)

4. Share two interventions a PCHN can implement to reduce or improve health outcomes for this chosen population. Please explain how he/she will do this with specifics. Do not just list interventions without providing detailed steps. Make sure you state what type of role you have (school nurse, public health nurse, outreach nurse, community health, etc). Your reading from Chapter 9 in Harkness and DeMarco should help with this. 1 paragraph (200 words)
   Support your post with at least one scholarly, peer-reviewed article
   Please remember that you need at least one paragraph per criteria and a paragraph is defined as 5 sentences or more.
   Responses (3 of them, each 300 words)
   For your peer response posts, choose 3 populations that are not in your own thread and answer the following for each. These should not have the same information in your peer posts since you are working with 3 different populations in 3 different locations,
5. (Paragraph 1) Discuss the role of the interdisciplinary care team/shareholders for this population. Who are they? Why are they important and what will they do? (150 words)
6. (Paragraph 2) Discuss two current resources from the local community that offer services for this population. Don’t just list them. Discuss how will they help this population. Use the zip code of your peer’s practicum facility to determine local community. (150 words)
   The citations and references must be peer-reviewed sources from the Delhi library, Google Scholar or another academic source and be listed in APA format. They should follow the basic rule of not being more than 5 years old.
