Practicum Evaluation and Reflection

This is the final assignment for evaluation and reflection of the practicum experience, covering the final two weeks in the practicum setting, Weeks 5 and 6 of the course. Once again, the basis for this assignment is your Learning Contract. Using the plans already present in Columns A-C, you are completing Columns D and E for this assignment as well as the other required elements as identified below and in the grading rubric, due at the end of Week 6.
Each update again provides a summary of activities, outcomes and their evaluation, and your reflection on the practicum service-learning experience in Column D. Column E logs your time again, and you will upload the all-important evidence for each week contained in the assignment in separate appendices (more information regarding appendices is below).
Here is what each weekly column that is due for this assignment (Weeks 5 and 6) should contain:
Column D: Outcomes, Evaluation, and Reflection (maximum of 250 words for each week’s content) – Answer the following:
Were you able to complete the week’s activities? Was the outcome as expected or did you find something different than what you anticipated? Did you meet your outcomes as expected? If your outcomes were not met as expected, why not? Have things gone as expected or differently? Are there any changes that you will need to make in your plan based on how the experience unfolded? If things did not go as planned, how will your timeline need to be altered in the following weeks? Do you realize that things should have been done in a different order?
Note: There is an option for Column D to use Jing, creating a screenshot of this document and/or other practicum documents to provide your outcomes and evaluation, and/or reflective journaling using audio instead of a typed narrative. Each column’s Jing is limited to 5 minutes, and instead of written content, you will supply the screencast.com link in the applicable column. The content will be graded according to the same rubric.
Column E: Time Log
• Log the hours, day(s), date(s), and total for each week covered by this assignment.
• Keep a running total of all hours completed. The minimum of 35 in-setting practicum hours must be met by the end of Week 6 of the course, with 45 total hours by Week 7.
Service-Learning Documentation: For this section, identify how you have contributed to the setting and increased your own learning as part of Service-Learning over the time frame covered. Specifically address how you contributed and what you learned through the S-L aspect of your project.
Synthesis Statement: Include a summarizing sentence on your practicum, for closure and synthesis, applying what you learned throughout your project experience to your personal nursing practice.
Required Appendices: An Appendix is supplemental material to a document; therefore, instead of adding this material directly onto the Learning Contract (which could create uploading problems), you will post each weekly Appendix separately entitled by the corresponding week: Appendix Week 5 and Appendix Week 6.
Column C has the evidence of accomplishments listed, and this content is what needs to be displayed separately for each week. The information provided should reflect the “evidence” that was stated in the Learning Contract. The content for the appendix can be posted, scanned, embedded, or in whatever format is appropriate to the item, but must be able to be viewed by your instructor as a separate document for each week. The contents of the Appendix must be explained within each week’s Appendix, and this can be done by either a written explanation or using Jing. The links for the Jing would be posted as part of the Appendix.
The assignment will have two (2) appendices, one for each week covered by the Learning Contract.
