Patient Safety

Each discussion question (1, 2 & 3) is to be at the minimum 600 words each, with cited reference to the reference included in the file. The reference section is not included in the 600-word minimum word count.

Week 1- Question 1
Please describe the attributes of a patient safety culture from the assigned readings or additional articles you have read from the literature.

Week 1- Question 2
From your own work environment or a work environment you have observed or read about, describe an example of a problem with multiple underlying contributing factors using the Swiss Cheese model. Optional but strongly recommended: Create an illustration of your Swiss Cheese model and attach it to your Discussion Question post. I have attached a blank “Swiss Cheese Diagram” that you may find helpful to create this.
Also the Swiss Cheese Diagram I thought would be an excellent addition to the discussion question like the discussion and the diagram.

Week 1-Question 3
From the literature or your own experience, describe where a Plan-Do-Study-Act (PDSA) cycle was utilized to try to improve a problem in healthcare. Be sure to describe the metrics utilized to assess whether the intervention (s) was/were successful in addressing the issue. Sub-headings or paragraphs in your post may include a) Plan, b) Do, c) Study, d) Act, and e) Metrics/Measurements. Try to describe whether your metrics represent a “process measure” or an “outcome measure” or both.
