Community Intervention

This is not a traditional discussion, so please follow the directions carefully:
Your initial post should include the following:

1. The chosen primary problem for your community that you, as the PCHN, will be proposing as an action plan for in week 7. Please provide a brief description of why this is your chosen primary problem and keep in mind that this problem topic will also be used for your intervention paper in week 5, and your live presentation in week 7.
2. One correctly written goal for your community intervention/s using the Logic Model on page 165 of the Harkness and DeMarco (2016) text.
3. One PCHN intervention that will address your problem using the SMART objectives on page 166 of the Harkness and DeMarco (2016) text including a timeline and measurable outcomes.
4. A brief discussion of how your intervention/s might be measured to show that your intervention was successful and that your goal was met (For example, detailed information about tools like a quiz, BMI measurements, number of narcan kits used, number of residents serviced etc).
   After the initial post, you will receive specific feedback from your instructor. Instructor feedback will occur by Wednesday, 2359 EST. You will take one of two actions, based on the content of the feedback:

• If the instructor provides feedback stating your problem, goal, intervention and evaluation are well structured and are ready to be used, respond to the instructor that you have read and understood the feedback.
• If the instructor provides feedback that indicates your goals, intervention and measurements need further development, then edit the areas for improvement and post the updated content back to the instructor.
  Although you will be able to see your classmates’ posts, in addition to instructor feedback, you are not responsible for responding to any of your classmates.
  Grading is 100% for full participation, demonstrated by items 1, 2, 3 and below and is worth 5% of your total Assignment 2 grade. The traditional discussion rubric does not apply to this week.
  It is expected that students will submit all 4 components of the assignment on time, by Monday 2359 EST and will either re-post edited content or acknowledge instructor feedback . Late policy of loss of 10% for each late day will apply.

1. Provides a brief description of problem of community and why chosen.
2. Accurately develops 1 goal according to the Logic Model on pg 165.
3. Accurately develops 1 PCHN intervention according to the SMART objectives on pages 166 in the text.
4. Briefly discusses potential methods for evaluation of each intervention.
