Case study: Tyrone is a 37-year-old male who presents with his mother to the psychiatric inpatient unit for assessment and possible admission (4th in the last 2 months). Tyrone has been diagnosed with schizophrenia and he presents as he has on previous admissions, delusional and psychotic. He is paranoid that his parents are trying to harm him and has stopped eating and drinking anything that is not sealed in a can. He will eat canned foods but only canned foods that he buys at the store and keeps in his room to ensure his parents haven’t tampered with it. On his previous inpatient admissions, Tyrone was reluctant to take medications and received several PRN doses of IM Haldol (which helped his psychosis). Near the end of his stay, he would be willing to take his oral medication and was discharged home. Once he got home from the first admission, he quickly stopped taking the Risperdal 3mg at night that was prescribed in the hospital. On the 2nd admission he reported that Risperdal “made me fat.” He was then prescribed ability which he stopped soon after being discharged the 2nd time. “I didn’t like it.” On the 3rd admission a long acting injectable was discussed but “There’s no way I’m taking a shot.” He was prescribed Seroquel, symptoms cleared and Tyrone was discharged home. His mom reports she isn’t sure he even took 1 pill of the Seroquel once they got home.
