  • A SMART Goal
  • The evidence demonstrating that improvement has been observed
  • Strategies
  • Action Steps
  • Timeline

The design and data must reflect the local context for the work.  Proposed evidence-based solutions to a problem must be identified to address the specific needs of the location.  Generic strategies or strategies employed elsewhere may not yield the same outcomes in a particular setting.  A sample template for the improvement plan is attached, though use of this specific template is not required.
